PROGRAM DETAILS


The Care Plan is a tool that creates a picture of your resident and should reflect all aspects of his or her needs, concerns and strengths. Proper identification of the resident´s needs is required to ensure that a comprehensive care plan is developed. An interdisciplinary approach is crucial to ensuring that none of the resident´s needs go unmet. Facilities that fail to accurately assess and plan for these essentials compromise quality of care and quality of life.



This one-day seminar will assist the participant in understanding the process that drives the care plan. Utilizing the MDS and RAPs, attendees will be able to write thorough interdisciplinary care plans. How to customize care plans to meet the needs of each resident will be discussed, as well as how to assign priority to a resident´s needs. Participants will learn how to incorporate other disciplines into meeting the resident´s entire needs and determine exactly who should be on the care plan team. Also, this seminar will address activity tag numbers F248 and F249 with CMS´s new focus on meeting the needs of the residents by the entire staff. Attendees will leave armed with the skills to write a comprehensive care plan, encourage participation from all care plan team members and improve their residents´ quality of life.

ABOUT THE SPEAKER


Gloria Gram, CTRS, is a Certified Therapeutic Recreation Specialist with 19 years of experience in long-term care. Currently, Ms. Gram is the Director of Therapeutic Recreation for Shady Grove Nursing and Rehabilitation Center in Rockville, Maryland. She has also worked as the Administrator of the Crescent Cities Adult Medical Day Center in Oxon Hill, Maryland, and as a Long-Term Care Consultant for Comprehensive Therapeutics in Northbrook, Illinois. Ms. Gram received her bachelor´s of science degree in Therapeutic Recreation in 1987 and obtained her certification as a Certified Therapeutic Recreation Specialist that same year. She is a member of the American Therapeutic Recreation Association and a former Regional Representative and Vice President of the Illinois Activity Professionals Association. Ms. Gram has spoken both locally and nationally on topics such as Psychosocial Survey Requirements, MDS, Programming for the Well Elderly and Therapeutic Recreation Programming. Her experience helps make her a knowledgeable and skilled instructor.
